Template for educational document T+Cs

I’m publishing a small e-book of 26 pages “The Clifford Evans Piano Method” and can’t see a suitable template on your website. It will also be put on my website clifford-evans.com

My aims are as follows:

  1. That all can download it free of charge
  2. They can distribute it to others without payment
  3. They are not allowed to sell it.
  4. They are NOT allowed to alter or add anything to the e-book
  5. I retain the copyright and the right to turn it into a commercial product at any time
