Consent for an author to use my name in his book

He has used my name in his book. Is there a consent form he should sign of me granting him permission to use my name? I would like him to use my name as I feature in some of the episodes.

There is no general rule of (English) law requiring consent for the use of an individual’s name in a book, although depending upon the exact circumstances the laws covering data protection, confidence, defamation, contempt of court and/or official secrets may apply.
